Yes. The game detects that you're cheating and that the data you send is different from the data you receive and forces a draw. This happens in PvP and to minor extent in Co-Op too. I would advise not cheating too much in PvP as that's where you risk getting banned

It could be related to the fact that the mod isn't yet updated to be compatible with Android 11. It crashes on Android 11 so I assume it's the same for Bluestacks. You should try using an older version and see if it still crashes.

Of course not. Most of the time when you play co-op you're playing with people which aren't using the mod. Just remember not to overdo it, if you want to use "unlock all challenges" it's absolutely fine. You can go for 2x damage and def if you want, there's a risk the game might force a defeat or be laggy, but as long as you don't overdo it there will be no risk of ban

I'm not 100% sure, I think they changed how this works, but there used to be 2 ban warnings before the permanent ban, but I heard now they just instaban you. Once banned, you cannot play the game anymore. Your account is unusable forever, can't play anymore.
Most people who get banned are at least 10k+ in PvP, so be mindful of cheating in that gamemode.

See if your device has a memory card
If yes, try removing the memory card
Restart and re-enter the game bro
