You have to die, literally that's the only way you can out there, happened to me once.
Just let yourself die of hunger/thirst.
You killed the boss too quickly, you need to kill all the mini-boss first, otherwise, you will get stuck.

Just use god mode. Not good kill the boss with one hit kill. And map in the most north, deactivate fast loot mod or else boss wont appear!

Only Aissa's event didnt appear.
About Traitor Catacomb, you need clear floor 1 dungeon of Torturer. And catacomb will appear on the map after you go home.
Does anyone here know how to trigger Aissa event?
